The State of Illinois requires all applicants for drivers examination to have a Social Security number and proof of date of birth, with verification of certified birth certificate or passport. All diabetic or heart patients must have a letter from a physician stating they are physically qualified to drive an automobile.
First time applicants must have a certified birth certificate, original social security card and proof of Illinois address.
